New Driver ID Request Form

State auditors have required ND State Fleet to implement a secure driver ID system to help defray fraud. University of North Dakota employees using a State Fleet fueling card to fuel a vehicle will be required to fill this form. A unique driver ID will be issued in their name.

Verify with the employee that they do not already have a Driver ID assigned to them from previous employment with the State of North Dakota or the North Dakota University System.

To have an employee removed from the Driver ID program please fill out the Driver ID Termination Request form.
