Driver ID Termination Request Form

ND State Fleet requires Driver ID's to be terminated when an employee is no longer employed by UND or the State of North Dakota. To have an employee removed from the Driver ID program please fill the form below. The card should then be destroyed. If the employee is continuing employment with the State or NDUS in another department, contact UND.transportation@UND.edu with the new department number and employee name - DO NOT TERMINATE THE DRIVER ID!
