Door Access
The door access form should be submitted by an authorized contact when an employee requires access to a room or door.
Lost or Stolen Door Access
The lost or stolen door access notification & replacement form should be submitted immediately if a door access device is lost or stolen.
Vendor Authorization
The vendor / contractor authorization form is used to request temporary electronic door access for a vendor or contractor.
Departments should notify the AS Technology department when an individual's access to a room or building needs to be removed. Notification can be made using the EDA email address und.eda@UND.edu.
